BELLI SCHOLARSHIP
John, Lynne and Nicole Belli Memorial Endowment Scholarship Program |
APPLICATION REQUIERMENTS
Applicants should submit a completed application form, along with the following items:
For current High School students:
- An official high school transcript (to include credits, GPA and SAT/ACT scores) sent directly from the institution; or an official copy of GED, if applicable.
- A copy of the acceptance letter/enrollment verification sent to the applicant by the university, college, or technical college.
For current University, College or Technical College students:
- An official transcript from the institution at which the student is currently enrolled, sent directly by the institution.
- A most recent invoice/billing statement from the applicant’s university, college, or technical college.
For applicants currently not attending High School or University, College, or Technical College:
- An official high school transcript (to include credits, GPA and SAT/ACT scores) sent directly from the institution; or an official copy of GED, if applicable.
- If applicable, an official transcript from any university, college, or technical college the applicant has previously attended, sent directly by the institution.
- A copy of the acceptance letter/enrollment verification sent to the applicant by the college, university, or technical college he or she plans to attend.
For ALL applicants:
- An official listing of costs detailing tuition, fees, room and board, books, and supplies.
- At least two letters of recommendation that support the applicant’s scholarship, work ethic, character, work/volunteer experience, citizenship, leadership, etc. (Recommendation letters may be from school staff, counselors, administrators, teachers/professors, employers or community members. Letters may not be from family members. Recommendation letters must be received along with the application in signed, sealed envelopes or must be mailed directly by the individual submitting the recommendation.)
- Validation of severe burn injury hospitalization. (Applicant is to obtain the documented dates of inpatient hospitalization from the hospital at which they were treated.)
- Required essays (described below).
Essays
Applicants must submit four (4) essays answering the questions listed below.
- Essays must be typed/word-processed, be in 12-point Times New Roman or Courier font and must have one-inch margins.
- Each essay is not to exceed two (2) typed, double spaced pages.
- Each essay should be submitted on a separate sheet of paper.
- Please include the number of the essay question and type the question at the top of the page.
Essay #1: How has your burn injury influenced and impacted your life and who you are today?
Essay #2: How will you benefit from your university, college, or technical college education and how will it contribute to your life after graduation?
Essay #3: Why do you think you are a deserving scholarship recipient? (You may reference your life experience, recent community service experience and future plans.)
Essay #4: What do you think are some of the most difficult challenges burn survivors face? What do you think are some of the most rewarding successes burn survivors experience?
DEADLINE: Scholarship application and all supplemental application materials (transcripts, letters of recommendation, etc.) must be received by:
- October 26, 2007 -- If enrollment date is Spring or Summer Semester 2008
- April 28, 2008 -- If enrollment date is Fall semester 2008
Scholarship recipients will be notified by November 30, 2007 or June 13, 2008.
Scholarship recipients will be featured in the Georgia Firefighters Burn Foundation newsletter and will be recognized on the GFBF website.
